The portal uses phrases such as “observed signs consistent with…” rather than labeling a person with a psychiatric diagnosis. Chaplains may document concerns and make referrals, while diagnosis belongs to appropriately licensed clinicians.

Mental-health reference definitions are educational and support clearer reporting; they are not diagnostic tools.

Use open, respectful questions; obtain consent before prayer or religious intervention; avoid interrogation; and do not pressure a person to disclose trauma details. In a safety concern, direct questions about suicide or harm are appropriate and should be followed by site protocol and clinical/emergency referral when indicated.

It is not a patient HIPAA authorization form and does not by itself establish that HBIU is a HIPAA covered entity or business associate. Chaplains must follow the privacy, security, documentation, and incident-reporting requirements of their clinical/placement site and applicable law.

Privacy & Confidentiality Commitments

I will follow the placement site's HIPAA/privacy and confidentiality policies. I will use de-identified case IDs in HBIU supervision reports unless HBIU has an approved secure workflow that permits otherwise. I will access protected/confidential information only as authorized for my assigned role. I will not photograph, text, email, or save PHI to personal devices/accounts unless specifically authorized by site policy and security controls. I will discuss confidential information only with authorized persons and in appropriate private settings. I will protect screens, papers, notes, passwords, badges, and devices from unauthorized access. I will promptly report a suspected privacy/security incident according to site and HBIU policy. I understand that confidentiality obligations continue after the encounter, placement, or training period ends.

Minimum-necessary & supervision practice

When the HIPAA minimum-necessary standard applies, regulated entities generally limit uses, disclosures, and requests for PHI to what is reasonably necessary for the purpose. Clinical trainees should also follow role-based access and the host site's policies.

For production use, HBIU should have this form reviewed against the requirements of each clinical site, its own legal status, applicable U.S. federal/state law, and relevant international privacy law where chaplains serve. Reference: HHS HIPAA Privacy Rule and HHS HIPAA Security Rule.

Mental Health Reference Library

Educational reference for chaplain documentation. Describe what was observed and use “may be consistent with” or “raises concern for” language. Do not tell a care recipient they have a diagnosis unless a qualified clinician has made that diagnosis.
